i skidded out of my lockring today into an intersection, into oncoming traffic nearly killing me, this was the second time on this same wheel, it is a formula hub, cog, and lockring. where is the problem? my previous formula hub with dura ace lockring never had this problem. i had my lbs fix it the first time. could i buy a new lockring and fix this?

cog isnt tight enough. rota fix that fucker on there, then retighten the lockring.
